BEVERLY HILLS, CALIFORNIA: Born with facial abnormalities, Nate Gomes requested a rhinoplasty from Dr Paul Nassif in the reality show 'Botched by Nature' in 2016. However, after a few years, the tip of the nose collapsed once more.

This time around in Season 8 of 'Botched', Dr Nassif replaced Nate's previous bone graft with a new one to build out Nate's bridge and widen his nostrils. This procedure helped Nate's breathing, leaving him with a more symmetrical nose.

Hailing from Merced, California, identical twin Nate was born with twin-to-twin transfusion syndrome and had undergone 25 surgical procedures.

The recent surgery by Dr Nassif was the most crucial since it allowed him to finally close this chapter and hopefully start afresh without having any more procedures.

"My nose is the most normal-looking it's ever been," Nate said. Nate revealed that he now finds it easier to go out and he is happy not being the focus of attention and not being stared at.

Nate said that he liked the way his nose looked when Dr Nassif operated on it five years ago, but the tip of his nose collapsed a few years later. The left side of his nose swelled, he couldn't breathe properly, and his wife complained about his snoring.

His nose is more symmetrical now, he can breathe correctly, and four months after surgery it seems to be a normal nose.

Nate is an identical twin born with a number of issues, including a hole in his heart, a cleft foot, and facial abnormalities. Prior to appearing on the show, he had undergone 21 surgical operations and spent more than $2 million to look more like his brother.

Nate sought out doctors Terry Dubrow and Paul Nassif in an episode of 'Botched by Nature' in 2016. "I was born with a handful of problems." Nate informed the doctors, "A cleft foot, a hole in my heart, my eyes were pretty far separated and basically looked like I had two noses."

Although identical twins, Nate was the only one born with the abnormalities.

During 'Botched by Nature', Dr Nassif had warned Nate that the treatment would be difficult. Nate's surgery lasted more than eight hours, with Dr Nassif acknowledging that he had put Nate "through the ringer."

He stated that he tried everything he could to improve his profile. "I worked on his chest, his neck, his face, his nose, the eye area, everything. So I hope he's in good spirits."

Nate was thrilled with the result of the surgery after a lengthy rehabilitation. His nose looked normal following Dr Nassif's initial operation. His nose was symmetrical, and he had no reservations about walking in public.

However, around the third year following his operation, Nate began to notice a few differences in his nose and breathing.

He realized that skipping the 2-year post-op meeting with Dr Nassif was posing a greater problem than he had anticipated. His wife, Brandi, has also expressed concern about his snoring. He knew that it was time to visit Dr Nassif and attempt to resolve his difficulties before it was too late.

During his consultation, Nate explained to the 'Botched' physicians how, a few years after surgery, he observed the tip of his nose cave in, and breathing through the right side of his nose had become difficult. The left side of his nose also began to swell and droop.

Breathing had taken precedence over sleeping and exercising. Nate had started gasping for air since his airways had become tiny. Nate requested Dr Nassif to try and repair the tip and shorten his nose on the left side.

According to Dr Nassif, Nate's nose had both aesthetic and functional concerns. His objective on the functional side was to widen the nostrils, and potentially add some cartilage to open Nate's airway.

Dr Nassif also suspected that some of the cartilage within Nate's nose had calcified, causing the fullness. Dr Nassif decided to operate on Nate using a closed approach, which implies just incisions within the nose.

Dr Nassif said that he was worried about performing the incision again because it was Nate's fifth rhinoplasty, which could result in the collapse of the whole tip.

Dr Nassif performed a closed rhinoplasty on Nate by creating an incision within his nose. He removed the scar and calcified rib from his prior treatment, which was causing his nose to collapse.

Dr Grace Lee Peng constructed the cadaver cartilage transplants and Dr Nassif employed a diced cartilage glue graft to correct his saddle nose deformity and increase tip projection.

Dr Nassif described how he removed a large amount of internal impediment to assist in opening Nate's airway.

Nate admitted that his nose appeared slimmer and more symmetrical, particularly at the tip. He was excited to see what the outcome would be after his nose healed. Dr Nassif requested that Nate not skip any subsequent follow-up sessions post-recovery.

She was invited to pursue a fellowship in Beverly Hills with Dr Nassif, Dr Babak Azizzadeh, and Dr Guy Massry, where she finished a year of training.
